its not always between the holes, it is between the holes most of the time because of the hole spacing. you have one hole, then like 10 holes' space worth of belt between two holes, so the probability that the perfect fit will be at some point where a hole lies is unlikely, and much more likely for it to be at a spot where there isn't a hole.
