Mutations mostly, but also people with compromised or unusual immune systems may not create the correct antibodies to fight it in the future. Mutations are why flu shots don't generally last more than a season.
IM vaccines contain a dead disease (the cheap oral ones used in the third world have a weakened, not dead, version, which is risky) so that your immune system can create antibodies to fight it while not being under direct attack.
The point of herd immunity is that if enough people are vaccinated to prevent a disease, it's unable to effectively enter a population and infect people who can't be vaccinated or are immunocompromised.
