Clearly those three things are the only things that could explain the situation. Can't thing of a single other thing that sets "christian countries" apart. Again, you're equivocating on "proof" and "reason to believe". I have good reasons to believe modern medical practice is effective: there's coincidence of medical practice and good health in directly accessible experience, there are artifacts of medical research which are difficult to explain without reference to an effective medical research apparatus. I don't have undoubtable proof of these things in the same way I don't have undoubtable proof that I have hands. I just have convincing evidence, which is the best you're ever going to get for any proposition about the world.
