I can't, don't have much occasion to use VPNs so I don't really know what's good or isn't.
As for the second question, there are two ways of getting an IP black listed, the most common is that a moderator or something will ban any IP address where abuse comes from, so if someone is abusing your services you play whack-a-mole for a while. When that happens re-rolling identities a couple of times will usually get you one that works. The other way is by analysis of the TOR network itself to find a nearly complete list of exit nodes that will connect to your service. Many of the IPs on the list will have never have been used for abuse but they'll be blanket banned because they all have the potential to be. It's a somewhat expensive operation so usually you do it at some time interval and accept new exit nodes joining the network will be able to access your service for some t less than your re-check interval. It's this second situation that probably explains what you're experiencing.
