There's what's call blebs in the lining of the lungs, which are highly compressed air pockets, between the two linings. Some are small, some are large. When the lung gets struck hard, the air pocket ruptures and the escaping air pushes the inner lining of the lung into the lung cavity, creating less capacity for incoming air. It's quite painful and debilitating. The only way to get that air out from between the linings is a long needle and suck the air out a bit at a time, or have an operation to access it directly.
