You're aware it's traditional for the speaker to introduce the president with the line "It's my great honor to introduce..."...she didn't, she is responsible for the resulting rebuttable to her handshake offer.
A handshake is a personal gesture between two people, the tearing up of a document that honors dead US citizens in it among many other things is not a personal gesture when done by the head of congress in front of congress and in front of country.
