if you have php you can just create a forward.php and put your code in there, no frameworks or applications required. I haven't tried to do anything like that specifically for a long time so check this:
using libcurl is probably the best way to do it, I believe you'll need one page (form.php) for the form that submits to the next page (record.php) which writes the data to a file and uses curl to redirect to the actual page

another especially fun one I've been following is the UNRWA.
before they stopped keeping a public tally (December I think), israel had killed around 150 UN workers and bombed 60+ UN designated aid facilities. as per the previous wars, the UN gives israel the GPS locations of their sites to prevent attacks and israel just uses them as targeting co-ordinates.
israel later claimed that the UNRWA was an enemy organisation because several of their workers were either supporters or members of Hamas, and that they'd been arrested. UNRWA says they can't do intense security checks on all low-level workers and it's possible, but that they have been given no evidence or explanation.
now the UNRWA has come out and said no evidence or explanation has been given to this day, but that the condition of the workers who were released from 'interrogation' implied that the others only admitted to crimes under extreme torture.

basically Pfizer ran a massive fraud when getting the emergency provisions from the FDA for their COVID 'va'ccine' - they used an expensive and time-intensive method to precisely synthesize the M'RNA sequences for the INITIAL batch sent to the FDA for approval, then for production batches they used a much quicker and dirtier method that left plasmid DNA in the end product at rates up to 800x greater than legally allowed.
the plasmid DNA has now been demonstrated to penetrate cells and contaminate native DNA in currently unpredictable ways. what's more, affected cells are replicating and mutating the contaminated DNA to the point where some DNA found in the cells does not match either the native DNA OR the contaminant DNA found in the 'va'ccine'.
