Independent private bodies developed the vaccine and the FDA gave it emergency approval after a reasonable amount of safety testing because we were and are in the middle of a pandemic.
The vaccines are safe and everyone who can take them has the responsibility to do so, masks have always worked and the only thing the CDC ever suggested was not buying them up during the initial shortage because they were needed for medical professionals.

Some governments tried really hard but weren't able to succeed, and were in particular hindered by a new wave of anti-scientific and conspiratorial thinking that has been hijacked by anti-vaccine and extremist right wing elements that politicized mask use and tried to hinder scientists and doctors at every turn.
America has only itself to blame for its failure to respond appropriately to an emergent situation. China handled their end of it better than the US, they learnt quickly. This should be a wake up call for the whole world that the rise of the internet and social media has allowed pathological ideas and misinformation to be proliferated for nefarious purposes. And the way to combat that without endangering freedom of speech is for private platforms to take action and for public bodies to pump resources into education, specifically scientific education. In the US in particular, general scientific literacy is poor and this needs to be corrected immediately.
