i'm pretty sure you can do one yourself if you know what you're doing. i think those companies just automate the process for you.

The registrar gets the money. I believe registrars pay a set license fee to ICANN which is a non-profit and theoretically only puts the money towards continuing its own existence but no one really believes that and it's assumed they have some kind of tax evasion scheme type thing that gives big kickbacks to stakeholders. Registrars make their profit on the difference between licensing and income.
The whole thing is kind of retarded and would be worth getting mad over if domains were more expensive or if we had a better technical solution and ICANN didn't conveniently paint over the dirty secret that we don't.
