Carriers typically rotate phones in larger IP blocks than subscriber lines, some IPs belonging to whatever the popular canadian telecom company is are banned so if you're in the right region it's a dice roll on wether or not you're going to get that IP or not. -

IP ban means what is sounds like it means, you're banned based on your ip address, not based on the credentials you used to log in. Each time your IP changes, as it does periodically with tor each time your exit node changes, there's a chance that your new apparent IP address will be a banned exit node. -
