can easily be done, if they really wanted to stop people circumventing bans they could lock down protocols like tor and block major VPN providers -

just because it can be done doesn't mean it's effective to do so. a state can assassinate a person who's saying things they don't like or riots can be quashed with live ammunition; it often just draws more attention to what they were trying to suppress in the first place. -

Run high speed port scanner on random IP ranges from first compromised mule locates port 27374 (Sub7) or what-have-you, log into discovered mules with master password, run applications remotely on mule-2-mule network. Almost impossible to trace back, if you use enough of them.
