Pretty sure it's either family or some systemic/institutional setting most times, the simple fact of the matter is that people that work with kids or are in close proximity to them have the most opportunity. We know that in order for a crime to occur we need to have motive, means and opportunity. If we suppose that 1% of the population has the inclination to sexually abuse a child AKA motive in this case, and 100% of them have the means, brute force, if you want to speak of rape. Then of course it follows that we see an over-representation where people in general would be in close proximity to children thus having more opportunity. "Stranger danger" is comparatively less common. But when it does happen it usually ends bad, as in murder.

Abuse is rampant in those group homes. My mom worked in a place like that and she abused the kids then came home and abused me. Jobs like that only attract the very most unskilled and under qualified people around. They have special training for employees about not abusing the fucking kids. I think it is higher than 1% because a lot of fucking kids get molested.

I think its one of the big issues that the media and state over exaggerate in order to scaremonger the population. then the general population all happily do what the state tells them and pay their taxes in return for being kept safe from the big bad dragons.
