The originator of the art/photo/item etc etc has to have had the intent to defraud for it to be a "fake".
I could steal a photo of your mom that you took and then post it on here saying "Here's a photo I took of my Asian love slave"...MY added subtitle would be fake but the photo of your mom wouldn't be would it? It's genuinely a picture of your mom!
I figured you'd have figured that out with a day to think about it...especially after you presented a photo of a movie prop (ET) and said it was a photo of an "Extraterrestrial Alien" ...I guarantee you the originator of that photo had NEVER claimed it to be a real alien...your subtitle was the faked part...
The "nazi" photo may originate from a movie/a TV series/Larpers having fun at the weekend...that's not fake unless the ORIGINATOR STAGE IT AND THEN PROPOSED IT WAS AN ORIGINAL PHOTO OF A WW2 NAZI KILLING SOMEONE"
Some random website "debunking It"...didn't debunk it unless they have that critical piece of information...what was the photographers intent?
